Individual Family Service Plan (IFSP) is written once a child is found eligible for services. The IFSP is an agreement between the family, service coordinators, and the service providers. During the IFSP meeting parents will be asked to prioritize their goals for their child since the parents will be developing goals they would like to see their child achieve.
During the IFSP meeting the family and the service provider will determine how much support they need from the Early Intervention team in order to achieve the set goals. The amount of support needed will determine the frequency of home visits with the Primary Service Provider and the family. The frequency of services, and the goals set during the IFSP meeting are individualized for each family. The IFSP must be reviewed at a minimum every 180 days, however it can be reviewed, and updated at any time at the request of a parent.
