A service consultant will be assigned to all those who are eligible and wish to receive services. Their job is to coordinate and monitor the services and supports each person receives, provide information, and refer individuals and families to community resources. All services offered by The Union County Board of DD are voluntary.

  • School Age Services (Ages 6 to 13)

    A service consultant will work with eligible children to:
    • Develop an Individualized Service Plan (ISP) to identify and address needs
    • Provide support at IEP (Individualized Education Program) meetings if requested by the family (IEP meetings are implemented by the child's school)
    • Identify needed resources and help link families to those resources
    • Identify funding sources
    • Help families advocate for services
    • Provide information about how to plan for the future
    • Other services as needed
  • Transition Age Services (Ages 14 up to 22)

    In addition to school age services, transition aged students may be assisted with the following:
    • Higher education opportunities
    • Employment opportunities
    • Identify strengths and needs and assist in finding resources to prepare individuals for transition to adulthood
    • Future planning (including transition from school to work, vocational services, day programming, driving assessment, guardianship, Social Security, Medicaid, voting, and other services as needed)
  • Adult Services (Ages 18 to 22+)

    Eligible adults can receive support from a service consultant to help with:
    • Developing an Individualized Service Plan (ISP) that encourages self-determination and identifies and addresses needs
    • Employment opportunities
    • Supporting adults in becoming as independent as possible by enhancing skills, eliminating barriers, accessing and developing opportunities to be part of a larger community, and help assuming adult responsibilities in the life they choose
    • Assistance in seeking funding options
    • Provide information about future planning, vocational services, day programming, driving assessment, guardianship, Social Security, Medicaid, voting, and other services, as needed
